SUMMARY OF BENEFITS AVAILABLE TO REGULAR EMPLOYEES


PAID TIME BANK (PTB): The Paid Time Bank provides compensation for short term or intermittent absences of a personal nature. It is a combination of holidays, personal days, sick days and vacation days. The amount of accrual depends on length of service, employment status and the individual entity. There is complete flexibility in using the time as needed.

When Eligible: Accrual begins immediately. Access times vary and are dependant upon each entity.

HEALTH INSURANCE: Eligibility after one month

Full-time and part-time employees may choose from among up to three medical plans. Premium costs vary with the plan, level of coverage, employment status and the individual entity, and will be deducted from each pay before taxes. All health plans offer a prescription benefit.

DENTAL INSURANCE: Eligibility after one month

Preventive and simple restorative dental procedures are paid under a schedule.

SHORT TERM DISABILITY(STD): Lancaster General provides employees with STD coverage at no cost.

When Eligible: Accrual begins immediately. Employees may use STD after 3 months of continuous employment. Access times vary and depend on the policy of the individual entity.

Unlimited Short Term Disability (STD): Employees who accumulate 480 STD hours will be awarded Unlimited STD. This means there is no need to replenish the STD hours that are used. Unlimited STD will be available for as long as the person remains employed by the Lancaster General health system. (STD applies only to the first 90 consecutive calendars days of a personal illness and LTD applies to eligible employees after 90 days.) Paid maximum of 480 per year.

LONG TERM DISABILITY (LTD): Eligibility: After one year of service for employees assigned to work 32 hours or more per week (.8 FTE). Lancaster General provides employees with LTD coverage at no cost.

PENSION: Eligibility is at age 21 after at least one calendar year of service working a minimum of 1,000 hours per calendar year. Participants with 5 years of service with at least 1,000 hours per year are vested.

LIFE INSURANCE: Eligibility after one year of service
Lancaster General provides eligible employees with coverage life insurance coverage at no additional cost to the employee.

VOLUNTARY LIFE INSURANCE PLANS & ACCIDENTAL DEATH AND DISMEMBERMENT:
Eligibility is immediate. Employees may purchase additional life and AD&D insurance for themselves and their dependents at low group insurance rates. Life insurance(s) are subject to evidence of insurability.

VOLUNTARY SHORT TERM DISABILITY: Eligibility after one month
Employees have the option to purchase disability income insurance. This benefit helps to safe guard income in the event of a prolonged sickness or injury when you have little or no STD time accrued. There is a one year pre-existing condition clause attached with 50% of benefit paid.

FLEXIBLE SPENDING ACCOUNTS: This program allows employees to use pretax dollars deducted from their pay in equal amounts to help cover eligible out of pocket medical/dental expenses and Day Care/Adult Care costs.

TAX SHELTERED ANNUITY: Eligibility is immediate

A TSA is an option to save for retirement with pretax dollars and earn sheltered interest at favorable rates.

TUITION ASSISTANCE: Lancaster General is committed to the personal and professional development of our employees. There are numerous opportunities including tuition reimbursement, classes offered by our Department of Professional and Educational Development, inservices, seminars and our college

FAMILY & MEDICAL LEAVE: Eligibility is upon completion of one year of service and working 1,040 hours.

Family and medical leave is provided by the Family & Medical Leave Act of 1993.

FUNERAL LEAVE: Eligibility is immediate
Employees are eligible for leave with pay for funeral of immediate family.

JURY DUTY & MILITARY LEAVE: Eligibility is immediate
Full-time and part-time employees may receive up to two weeks of full pay for jury duty on days scheduled to work. A leave without pay will be granted for military duty in accordance with federal regulations.

PHARMACY: Employees are given discounts on prescription drugs as well as a discount on over-the-counter items through the LGH Convenience Pharmacy.

VISION CARE: Eligibility is immediate. Discounted services, frames and lenses are available at specific locations.

EMPLOYEE ASSISTANCE PROGRAM: Eligibility is immediate. Confidential counseling services to help you and your family.

DIRECT DEPOSIT: Eligibility is immediate. All employees will receive their paychecks through direct deposit.

ADDITIONAL DISCOUNTS & BENEFITS: Eligibility is immediate

We offer a variety of other voluntary benefits such as personal cancer insurance through AFLAC, supplemental disability income insurance through Mass Mutual Life Insurance Company. Also, free parking, discounted meals in the LGH cafeteria, payroll deductions, free flu shots, and inoculations. The Employee Service Center is available on the first floor of the hospital. The Identification Badge machine is located in the ESC. Services available are dry cleaning, film processing, use of fax machine, copier and PC’s with internet access, discounted movie tickets, special show tickets, amusement park tickets and other coupons.
